It is important that both player and coach understand how each other work, and have the same desire to work hard and achieve goals. Please complete the form below to ensure that we understand your current situation, goals, and likes and dislikes so we can make the training and coach/player relationship as effective as possible.

I need to work with a disciplined, hard working, motivated player, who is receptive to taking advice, with a good attitude, having a relationship based on trust and openness. To get the best results for you, to improve your game and to work in a good atmosphere I need to know the information requested on this form:


What are your expectations?

How good do you want to be/what are your goals?
What are your ranking targets?
What is your time frame?

What are your expectations of your coach:
technical adviser, strategy, fitness, mental, plan schedule, negotiate contracts, enter tournaments etc.?

Will you make changes to your game to help you improve long term or not? How much pain can you take?

Training:

How much do you train each day on court?
What do you do during your training sessions?
Do you like to train with other players or the coach?
Do you have a fitness program – Strength/Speed/Agility/Endurance?
Do you have a mental toughness program? What is a normal training day: 1-Competition, 2-No competition

What is your competition schedule?
How much of the time do you want the coach to travel with you?

Are you self motivated or does the coach have to “push“ you?

Before match: how do you like to prepare, what information do you want?

After match: prefer time to yourself, or discuss match right away, reports on the match?

Do you like to tell the coach or must he figure it out?

Do you like to play from ahead or behind?

What do you need to work on? – technically, conditioning, mentally and off court?

What do you like to do to relax/off court? Do you like to socialize with the other players?
